Creative Community Connections
 With over 35 years of proven excellence in residential support to people with developmental disabilities, Toward Independence, Inc. (TII) is thrilled to announce the expanding of its services to include Creative Community Connections (CCC), a multi-faceted habilitation program. CCC is being introduced to enrich the lives of individuals by providing new opportunities for them to learn and develop to their full potential.
Three different programs are offered under the CCC umbrella:
- Adult Day Support
TII is a certified Day Habilitation provider. Under the HCBS Waiver Rule, we offer an alternative to traditional workshop/Day Habilitation services. To receive these services, individuals must be enrolled in the HCBS waiver and choose TII as a HCBS Day Habilitation provider through the Free Choice of Provider Rule (5123:2-9-11). In addition, the services must appear on the consumer’s Individual Service Plan (ISP). Transportation to and from Adult Day Support sites is also provided under the HCBS waiver.
- Spare Time Choices
Features a calendar full of events designed around a variety of recreational interests.
- First Class Opportunities
Structured classes give new opportunities to use and expand talents and abilities.
Services can be provided through the following funding sources:
- HCBS Waivers (I/O and Level I)
- Private Pay
- ICF/MR
